WHY THIS PROGRAM IS ESSENTIAL
1. Vulnerability During Transition
• Adolescents with SCD often lose access to familiar, coordinated care.
• Adult care systems are rarely equipped with the same level of specialization and psychosocial support found in pediatric programs.
• Studies show that morbidity and mortality increase significantly during the transition period.

• Young adults may stop engaging in care altogether due to fear, mistrust, or lack of support.

• Many feel unprepared to advocate for themselves or navigate the adult care system.
